Due to organisational growth a leading Australian Not For Profit organisation has the need to appoint a Care Coordinator to their business.
The purpose of this role is to work as a senior nursing clinician to provide a variety of generalist nursing duties, including medication management, assessment, coordination and case management plus supervision of staff to ensure appropriate care for residents.
Your responsibilities will b e inclusive of but not limited to the following:
*Provide clinical leadership and supervision to the day-to-day activities of care teams including RNs,EENs, PCW/AINs, DTs and allied health staff.
*Promote a culture of service delivery that aims to improve the lifestyle experiences of residents.
*Develop appropriate care plans in consultation with the resident, their family/carer and other members.
*Assist the Single Incident Management System ensuring all clinical and OHS incidents are notified and recorded and participate in any investigations as required.
*Assist with the management of clinical stocks across the facility ensuring availability and compliance within budget.
*Work floor shifts including shift work (when required)
*Ensure the provision of timely, accurate and reliable client care and clinical documentation.
*Ensure that staff in area of responsibility have the skill, ability and competence to safely carry out the duties of their roles.
Scope Of The Role:
*Five day week
*Good salary & conditions
*Ongoing training
*Aged Care experience preferred (2 years minimum)
*Current appropriate Nursing registration (essential)
*Demonstrated competence in clinical governance (essential)
*Computer literate in Microsoft Office Suite
*A Current open drivers licence needed
*A Federal Police Clearance must be attained
