We are currently looking for a very unique and skilled person who not only has extensive experience is case management, but who also has passion and compassion for working with individuals, to create, implement and monitor care plans! If that sounds like you and you have the ability to coordinate support services for individuals with mental illness and multiple needs, then please read on.

Our client is a consumer-focused organisation who provides a range of primary health and community support services. They require a Care Plan Coordinator to develop or recommend flexible, creative and sustainable service options for their clients. The Care Plan Coordinator will work as part of the Out Reach Mental Health Team.

To be successful in this role you will:

• Have the ability to engage and influence stakeholders and work in an inclusive manner • Have sound decision making skills with a focus on identifying and understanding issues, problems and opportunities • Have tertiary qualification in relevant discipline
• Have extensive experience in providing community based support within the human services sector • Have demonstrated ability to work effectively with clients who are difficult to engage and/or have a range of complex needs • Have understanding of the key issues for people with a psychiatric disability, drug and alcohol dependency • Have sound knowledge of the drug and alcohol sector and experience working with individuals presenting with drug and alcohol dependency • Have skills in crisis intervention and management
• Have demonstrated understanding of working within an assertive outreach model • Have skills in care coordination • Have the ability to work independently and as an effective team member • Have highly developed communication, report writing and interpersonal skills • Have a current Victoria’s drivers license
