Our client is a community health organization that is dedicated to providing culturally appropriate, accessible and integrated health services. They provide a range of community services to Inner Northern Suburbs of Melbourne. Employees take an active role in service planning and development, quality improvement and health promotion.
In this role you will be working as a Community Support Worker providing direct support to your patients in a flexible, individualized approach that is tailored to meet their specific needs. You will be providing activities to promote choice, acknowledge individual strengths and facilitate inclusion into the wider community. This support will be provided in the context of their friends, family and neighborhood. This will also be delivered within a team approach, allowing opportunities for reflective practice and for more flexible responses.
To be successful in this role you will have:
•Experience in providing community based support within the psychiatric disability support sector and to individuals with multiple and complex needs.
•Demonstrated understanding of the key issues for people living in the community with a psychiatric disability who have experiences co-existing substance misuse problems, coexisting health problems and/or intellectual disability and acquired brain, and relevant service policies.
•Familiarity with psychosocial rehabilitation and recovery approaches.
•The capacity to work in a team based service.
•Demonstrated ability to work effectively with clients who are difficult to engage and/or have a range of complex needs.
